Add handling duplicate code chunk names #46

Please document handling duplicate code chunk names in https://github.com/Bioconductor/sweave2rmd/blob/main/vignettes/troubleshooting-faq.Rmd. You can follow the question and answer format.

You can add some additional details such as they prevent knitting and also make BiocCheck fail.
