WIP: BIP draft #26

This PR is not intended to merge, it is just a discussion place for writing a draft of the BIP document. We also need to write up a mailing list post, but let's nail down the overall BIP structure first.

The first five commits just rebase Russell's ms32 branch on master (which applied cleanly)

Closing since we have a complete draft elsewhere, and never actually used this PR as a discussion point.
