refactor: move the custom fhir content to the `FHIR-Converter` repo #2856

Summary

Move the customizations of the FHIR-Converter tool into the skylight fork of the repo instead of housing them here. Corresponding PR

All C# code is moved out of this repo as well as templates.
Update README with current guidance
Make testing and build/make commands a bit more robust
