Red Screen of Death on Version 2.12U #256

Whenever I run the disc the console gives me the red screen of death and does not recognize the disc as a PS2 format disc. It also does not show up in the browser it just launches it and I get the screen immediately.

Nope it just gives me the "please insert a PlayStation or PlayStation2 format disc" every time
