I have not experienced this limitation, although I have not tried taking a party of NPCs into combat. My current character has 8 NPC followers, and they will all happily walk around with me or ride in my vehicle unless I order them to do otherwise.
So, they're not showing up as green dots on my minimap, rather pink. Even though i've done quests for them and got them to follow me back to camp their opinion of me isn't super high. They have additional jobs for me to do (and won't follow me until I do them i guess?) because there are 4 others that are green on the minimap and they DO follow.
I ended up debugging and finishing their quests quickly to see and their opinion of me didn't change, much. It seems like the NPCs that show up 'green' on the mini map are best friends and will do whatever you say, but the 'pink' mini map ones are indifferent and won't follow, even though they're in the camp and I can trade with them for free and such. Is that suppose to happen?
A save file to reproduce this issue would be helpful.
3npc.zip I parked next to the npcs that wont follow with the 3 that will already following me. They have no problem working at the camp and I've done all the jobs i can for them.
Confirmed using your save file. Note: I do not have Mining_Mod or cars_to_wrecks.
The two bugged NPCs are neutral, not hallucinations, no_faction, attitude: ignoring, value 15 and 20 (Best friends forever!). I am not sure how to check, but they appear to be guarding your faction camp. They do show up in the "your followers" tab of the [#] factions UI. On that tab their status is displayed as "nothing" instead of "Following" or "Guarding". Below the status it says "working at camp". If you talk to them, tell them "I need you to come with me" and then "Let's go", nothing changes.

Some areas in the game are just brutal and I have quite an NPC force at home, but it seems once you have three followers and tell more to follow, they just ignore you making raiding bandit's bases with lots of bandits a near impossible feat.
Solution you would like.
It would be nice if there was no limit to the amount of NPCs you can have following you. This would reward you heavily with doing all those npc quests, earning their trust, and then using them to take over fortified bandit areas.
Describe alternatives you have considered.
I'm not sure if three was a soft cap or if more was just to complex for the game, but like mentioned above, three is almost not enough for some areas.
Additional context
I use the no hope mod where bandits are everywhere and having more than three would be nice. Without that mod, you still have things like mi-go towers, science and military bunkers, and the like that could really benefit from a large party that trusts you and is armed! :D
