Update for pgbouncer 1.18 #11

New columns added to SHOW statements in 1.18

Add application_name to SHOW CLIENTS/SERVERS/SOCKETS output 
Add information about cancel requests to SHOW CLIENTS/SERVERS/POOLS output

Fixed in v0.5 and will also be handled better in upcoming 1.0.0
