Purge Alaska Specific Content #62

"Alaska Logbook" and "AK Logbook" and things of that sort are EVERYWHERE! Clean those out, find more subtle ones and remove them as necessary:

Some of these changes will need to be made in the Formhub (and maybe Enketo) repos, too.

Just need to remove aklogbook link from form as well as Alaska logo (enketo?)
