This means more clicks and the potential of not recording a date even if present due to misunderstandings of “unknown”. It would be fully OK to have the last year of visit recorded.

Change "Date of last data collection" so that the options are "Known" or "Unknown". If "Known" is selected, the date field appears with the field note "Most recent date that information was collected that has been used to complete the form. Use 01 (Jan) as month or 01 as day if said information is missing"

Ok, I was following through on Tony’s suggestion but happy to be guided by you. There may be cases where not even the year of last review is known so we need to have a way for people to enter unknown for the field overall. Should we just remove it as a required field then?

Can you try entering no last date and tell me what you think of the process - you can override mandatory fields. Also, data that goes to the study database won’t have those checks anyway.

I tried it and see what you mean – think it is fine to leave it as is and people can ignore the alert if needed. Thanks
