Update Kanta measurement lab outcomes into value concept id #192

This is for next release as part of Kanta improvement.

The idea is to capture TEST_OUTCOME and TEST_OUTCOME_IMPUTED column information from Kanta into value_concept_id.

For example - A test output is Abnormal then the value_concept_id will capture that.

As of now, the values in TEST_OUTCOME are not clear and needs better understanding and mapping.
