From quick test, it seems any item that contains Naquadria or Enriched Naquadah (oxide mixture dust included) is radioactive. I first found out when I removed Leggings of the Burning Mantle to avoid creating light source for the pure Naq bees (nocturnal-only apparently has a light level requirement on top of night time). The hazmat set is incomplete and I was subjected to radioactive poisoning from the Naquadria block.
Your Goal
There are radioactive tooltips for GT++ materials, so I suppose it is understandable that not every element may get the same treatment.
On the other hand, the quest book directly mentions the radioactivity of enriched naquadah in only one place: "Portable Clean Energy EV" introducing Naquadah reactor Mark I.
And mentioned less directly as one of the main radioactive materials used in HV Radio Hatch, a hatch for Bacterial Vat Multi.
Your Vision
I believe there lacks a tooltip or quest hint about the radioactivity of Naquadria and Enriched Naquadah. Further more, in quests it is mentioned in less common places.
