New term details

Term name - recent antibiotic usage
Structured comment name - recent_abx
Definition - Antibiotic usage in the prior 3 months. If yes, indicate if possible the date of the last dose
Expected value - recent antibiotic usage status
Value syntax - {boolean};{timestamp}
Example - yes ; 5/11/2018
Preferred unit - 
Package - Urobiome

Additional context Suggested as part of the Urobiome package.

Change structured comment name to recent_antibiotics. Make it boolean only.

Consider adding a separate field for time_since_antibiotic. See the farm animal packages for examples.
