Updates for appsody 0.5.3, exercises 5, 6, 7 and 9 #75

Update and test exercises for errata in boxnote from @timroster. Individual commits for exercise updates and errata from actual runthroughs.

Update screenshots and include output in exercise 9 for running application with correct headers.

Update version strings in examples for consistency. Current (and going forward) incubator/nodejs-express will be at 0.3 and later.
