The newest IHE template has a download.md, as you have. This should be called for in the sushi-config.yaml menu, as well as the index.md. See the IHE template for the current methodology. https://github.com/IHE/supplement-template

note also that the downloads.md has some template Figure headers that should be updated or removed when there is no table.

The supplement template has been updated. See that the figure stuff has been removed.
