Create page structure for Volume 1

Create standard empty pages for Volume 1. Use IHE template: https://github.com/IHE/supplement-template/tree/master/input Eventually add content already existing, e.g. https://build.fhir.org/ig/IHE/pharm-mpd/actors_and_transactions.html
