Survey setup bug: Assigning hierarchy levels to Respondents #204

When Facilitator adds Respondents to take a survey, they should select the Hierarchy Level of each respondent, not the Hierarchy Level List item.

For instance, I would want to add "svasdev@irex.org" and then "International NGO" (hierarchy level). I would not want to add "IREX" (which would be a list item within the "International NGO" hierarchy level).
