Respondent bug: Role question #214

When a Respondent is asked for their role, they should have the ability to add their own, if the list doesn't include it.

Per the wireframe, the question should be "What is your role?"

Really, what we want is for the question to say "What is your role at [insert list item of the lowest applicable hierarchy level]"?

For example, if the Respondent says above that they are at Nation: Tanzania, Region: Arusha, District: Arusha DC, Facility: Not Applicable, then the question should say "What is your role at Arusha DC"?

If this question needs to be moved to the top of the next page (so that their response to the earlier hierarchy level question can be added into the question itself), that's fine.

Update: Too complicated. Instead: "What is your role at your organization?"
