[X] Add Book Title to NinjaForms list #84

Ninja forms submission page should have the book title so that the list of volunteer submissions can be sorted by book (in order to compare and choose the best reviewer).

On the Ninja Forms "edit item" page where you get the individual form submissions, the book chosen on the form is not shown. (You can't tell which title the person wants to review.) The pull-down list of books does not seem to be books that are actually with the status "awaiting review."
