Remove lists of extensions from the top of the extension appendix? #64

Forwarded from the archived internal repo (https://gitlab.khronos.org/vulkan/antora/-/issues/9 per @tobski) - I think we missed porting this over here.

"There's currently very long lists of extensions at the start of the extensions appendix, which feels redundant with the in-situ table of contents generated on the Antora site - should we remove these lists for this version of the spec?"

FWIW, my (@oddhack) take is that this makes sense, and is easy to do. The right sidebar navigation will still be very long since there are hundreds and hundreds of extensions, but after the upcoming fix to move the extension appendix internal sections down one hierarchy level, it will be more navigable.

This is fixed in the internal Vulkan spec repo and will show up the next time we do a spec update - the lengthy 'table of contents' links are no longer included for the Antora build.
