Lookup shows only one table #13

I used the tool to create a polymorphic lookup using two tables, one out-of-the-box (Account), and one custom. The lookup is defaulting to the Account and not showing the custom table, even when clicking "Advanced Lookup." Both relationships are showing on my source table (another custom table) but the lookup seems to be defaulting to Account only.

Are you sur the custom table is included in your Model Driven App? Anyway, if any issue in the app, it's not related to the XrmToolBox tool itself
