AVF (mac & ios) fail to decode internal H264 test data #78

RainbowGradient.h264 is the only remaining one. This does decode, but has a different width from mediafoundation's output. https://github.com/NewChromantics/PopH264/blob/master/Source_TestApp/PopH264_TestApp.cpp#L574
