Custom header/footer php support #21

Whenever possible, it's best to use MediaWiki for customizing aspects of a site, but sometimes some custom headers and footers might be needed.

This topic was brought up in issue #20, and it's something I was contemplating this past week already.

There are probably two different requirements for this:

  1. Initial support for checking for the files, and including them if they exist.
  2. Provide some simple, useful examples for people to use.
