Add adapter documentation #18

The framework has two built-in adapters for FHIR and SQL. Both require YAML-formatted configuration files. We need a comprehensible documentation of both adapters.

In addition, we should provide documentation/ a starting point for implementing new adapters.

I added some documentation, but I don't really know what all the configuration properties are used for.
