I mustn't remove their support in patch release - this should happen in 2.5 only.
I have put back support for several hashes as well - they should be removed in 2.5.
restrict usage of second block hashing only if OpenSSL/LibreSSL/GnuTLS is available (since IMO I found its implementation useless for SSL libs, which will be removed in 2.5)

@michaelrsweet please review this once you have time - I will release 2.4.8 to fix support breakage between patch versions.

@michaelrsweet they were accessible via public API in the previous series of 2.4.x, so I would rather make it still available for old implementations (GnuTLS, SSPI and CDSA) to be sure, and mention their removal in 2.5.
