moved checklist up, rename "Cohort Planning Checklist" to "Cohort Setup Checklist"

@jules32 this PR involves only the "Pre-cohort tech setup" page. Have reordered things and edited text. I'm happy for now with content up to Cohort Registry.

I enabled section numbering to make nested headings more clear.

Happy to hear any feedback, requests for changes etc.

TODO in a new pull request after this is reviewed & merged: I'll edit the sections that follow Cohort Registry and add stuff on Google Meet etc.
