refactor: a more accessible page header #205

move the breadcrumb navigation and page tools into a <header> element.
only use one aria-current="page" on each page, preferably on a link.
mark up the page tools as a labelled list.
make the license attribution a button, so that it appears in the page tab order.
fixes #197.

WAVE isn't picking up the 'Sign in' label on the user avatar button. Other than that, the navigation links and buttons seem to be listed, in the page tab order, as expected here.
