Get death primary key sorted out #5

The PK on the death table is just (id) in the staging and warehouse dbs.

cause_of_death_concept_id is described as not required in the etl conventions, but it is part of the primary key on the death table according to pedsnetcdms and the standalone ddl scripts in data_models.
