This is not easy to fix. p4est has unclear smoothing, see https://github.com/dealii/dealii/pull/1670 And do proper smoothing in distributed mesh is also not easy, see https://github.com/dealii/dealii/pull/1676 and https://github.com/dealii/dealii/pull/1711

So, for now, this issue will no be fixed for the foreseeable future.
