Add reset and reset all instructions.

Rename QuantumState.measure to QuantumState.apply_measure. Add QuantumState.apply_reset and QuantumState.apply_reset_all.

Gates: fix compilation warning.

Do not merge this yet.

We need first to:

update QX simulator to use libqasm/0.6.7 (with support for the reset instruction), and
enable the tests marked as DISABLED_ and see they run without issues.
