Persist range state in plots #146

This ensures that both skyplot and scatter plot ranges persist when adding and removing metrics and when switching between simple and detailed views.

Additionally this redesigns how plots are instantiated a bit.

This does not yet handle persistence of state in a more general sense, which I'd like to postpone until https://github.com/holoviz/panel/pull/1150 is merged which will give us bidirectional control over query parameters in param.
