Cutie Pox, as described by Ms. Sparkle, was a "puzzling pony plague [that] afflicted a population of ponies back in the Paleopony Period! Random cutie marks appeared all over the ponies' bodies, causing them to perform all the talents that came with them! The cause of the breakout was never discovered, and the cutie pox disappeared as mysteriously as they arrived!"
Apple Bloom has a variant caused by over-consumption of Heart's Desire, but we can infer the symptoms of the original plague based on hers in the corresponding episode. Translating that to Minecraft, there could be a few minor buffs to certain skills, but coming at the cost of the infected player's hunger draining much quicker, and eventually draining their health as a result, before they inevitably die if uncured.
Like in the show, the plague could be cured by the flower that grows from the Seeds of Truth, which would have to be added as a item along with this illness. Heart's Desire could also be optionally added as an item with various other benefits if brewed properly, but cause this illness if eaten without brewing.
Feather Flu is an affliction that pegasus ponies suffer in the show. The illness likely has the same affects as the actual flu, but for gameplay purposes and referencing the episode it appears in (Hurricane Fluttershy) it could have a negative effect on the flight of pegasus ponies, either slowing their flight to severe degrees, or eliminating their ability to fly at all until they recover, as in the episode it prevents the sick ponies from participating in the hurricane flight at all.
It likely wears off over time, but it'd be fun to implement a cure of sorts in a potion that a Zebra pony or other skilled potioneer could brew. Soup would likely lessen the duration of the affliction.
Fur Blight, as mentioned in both Leap of Faith and A Health of Information causes a pony to lose their fur coat, and begin balding temporarily.
In game, this could make a player more susceptible to the elements, causing them to gain a freezing status effect simply by walking into a snowy biome, or coming into contact with ice blocks. They could also start taking fire damage in desert and savannah biomes without the fur to protect them from burns or insulate their body properly.
Hoof-and-Mouth-Disease, as mentioned by Pinkie Pie in Over a Barrel, is an affliction which symptoms we don't actually see or hear described. Luckily, the name is quite self-explanatory.
In gameplay, this could translate to slowness and hunger status effects. Alternatively, instead of a hunger effect, it could prevent eating entirely for a temporary period of time, or cause damage with every food item consumed in reference to perhaps a sore mouth or locked jaw. This would still come with the slowness effect. Unsure as to how one would contract it.
Apple Blight, a sickness mentioned by Granny Smith in Where the Apple Lies, isn't an illnesss that affects ponies, but I feel still holds a strong purpose in a system like this, and could be used even if a disease system as a whole isn't implemented. This is because it causes apples to begin to rot before they can be harvested, and eventually the tree begins to turn red, the red parts becoming unsalvageable and having to be pruned for the health of the tree.
In gameplay, this could result in rotten apples in an apple tree when attempted to be harvested by an earth pony. Eventually, if the tree is not cured in time, the trunk will turn red, and the tree has to be cut entirely, or risk infection of other healthier trees in a close enough area.

I would love to see a system that adds magical diseases and cures Though there not many illnesses depicted in the show and there usually less of an illness and more like magical plants like the poison joke plants or the swamp fever tree which turns other creatures into tree's if they get infected with swamp fever
