Create Standard Issue Templates for Projects

Issue templates are done in GitHub by simple Markdown files in .github/ISSUE_TEMPLATE. We could add some starter ones to the https://github.com/TBD54566975/tbd-project-template repo, and issue PRs into existing projects.
