Thinking about this without time or cost contraints perhaps the design would be a table where each row is a type of action and each column may have a checkmark. Actions shown as not done would only be there if the action had at one point been done.
Then, moving toward data vis, instead of check marks the columns could be lines. See sketch below:

I realize I just duplicated this issue, so I'll delete that. I would really like to make it less complicated!
If the action items that are entered can just flow into the same number of columns as there are years, or same number of columns as there are categories (whichever is smaller). No need to list actions that are not done, no need to reference years above. How does that sound? Should I post a sketch?
In using the site, we realize that the repetition of other actions for each year will likely pose a problem. If a company continues to take the same actions, it doesn't make sense that the same actions repeat each year (or don't repeat and it looks like they are no longer doing them). One solution could be to put only the latest year's actions, like how the page only has a link to the latest year's footprint report.
