What are the information that are expected:

Disclaimer "not a laywer": I doubt that Section 11. and 12. are possible for a paid service and can only be applied in combination with e.g. open source code if you look at a MIT license. Such liability exclusions cannot be part of "standard terms of service" but must be a separate. Also they cannot be too broad.
