Test::Builder still contains vestiges of its own output system. Mostly in making its own filehandles and assigning them to the formatter. It also still hangs onto the filehandles inside itself rather than asking the formatter.
Remove Test::Builder's own filehandle duplication and resetting
Make it rely totally on the formatters and streamers
Add reset_streamer() to the Formatter
Make TB2::Streamer::Print and TB2::Streamer::TAP hold stdout() and stderr() as class attributes so they are only duplicated once.
