create a national version of the demographics files (principal projection) #11

currently the demographic files are created for individual providers. we need to have a version of these files for the nation as a whole. Summing the provider files will end up sort of double counting though, so we need to access the raw data (by local authority).

for now, this will be only for the principal projection. over time we can build the other demographic projections

handled by https://github.com/The-Strategy-Unit/nhp_model/pull/214 currently. Will create a new issue for handling all of the other projections
