What mods and what IWAD are you running at the time? Also, how willing are you to help debug this issue?

Freedoom: Phase 1 or Freedoom: Phase 2. I'm willing to try to fix this issue by having Apple completely swap out my mainboard with a new one, so pretty much anything lol.
Since you say you can loopback the sound without problems we can certainly rule out a software side problem.

Well it's definitely not my hardware. I had Apple check everything, they even replaced basically my entire unit and this problem still exists, and doesn't happen with anything other than GZDoom. I made a new issue with updated info and stuff.
GZDoom version
v4.10.0
Which game are you running with GZDoom?
Doom
What Operating System are you using?
Mac OS
Please describe your specific OS version
MacOS 12.6.3
Relevant hardware info
MacBook Pro 2019 16-inch (i9-2.4 5600M)
Have you checked that no other similar issue already exists?
A clear and concise description of what the bug is.
After about 7 minutes of playing in GZDoom, the midi starts to pop a bit randomly, then it speeds up, and it speeds up until it reaches INSANITY. I bet someone could just take one listen to this and determine what it is: https://youtube.com/shorts/pTfqxNuypWs?feature=share
I've had this happen on my i9 that I had a year ago, then it also happened on my M1 Pro 16-inch, and now this i9. I will say though that it doesn't show up in audio loopback recordings that I do, so that's weird.
Steps to reproduce the behaviour.
I would have no idea how to reproduce it. It just happens almost every time for me.
