Update README instructions to match recent code changes.
Update README instructions for web + postgres to work in a fully containerised context. (NOTE: have not considered pipeline! Pipeline is considered outside the context of this PR)
Made several highly opinionated changes.
Added some helpers to Makefile.
Fixed a few spelling mistakes.

Slight pivot here, but since we are making a number of changes we should move this to the Wiki. This README is too big.

I'm going to agree with you - but don't have the bandwidth to do it right now. I'd like to start moving towards Wiki after merging this PR.
