Create output fcsv

This PR adds ability to convert from the probability segmentations of each afid to a standard fcsv file. To do this, I've included a fcsv template and an additional python script. The script does the following:

• Identify each afid from the segmentation image
• Compute centroid
• Convert centroid into world coordinates using image affine
• Replace template fcsv with computed coordinates for each afid
• Output an afid (that attempts to match the afids naming scheme).

I haven't tested this on real data yet, but the code from the script does work separately.

Cleaned up the repository and combined a few rules. Needs to be tested before merge!
