All games start with a title screen. You must click the mouse button or tap the touch screen to progress past this screen. As this is a text adventure, you shouldn't need to rely on a mouse. If you don't have a mouse on desktop (as is typically the case with visually-impaired players) and you don't have a touch screen, then you can't progress beyond the title screen.
