Checkout table cells

There seems to be a general problem with Magento 1.4.2 checkout table.
The labels and cells are mixed up. Product names appear in the "Wishlist" 
field, Qty in the Unit price... etc. All messed up.

On Magento 1.4.1.1 (checkout_table_m1411.png)
Tested on /magedev/

On Magento 1.4.2 (checkout_table_m142.png)
Tested on /mageimage/

Tested with Fixed Qty, common w2p product and a simple M product. Even the M 
simple product appears this way.

Needs to be tested on a fresh M 1.4.2 installation to confirm it is not caused 
by our extensions. Might be a M 1.4.2 problem?

Use mageimage for fresh installation and 1.8.3.2 (latest stable) release.

Can I reinstall M on /mageimage/?

I reinstalled the extension but still see the old checkout table cell order.

It does have all of those "complex products" setup for FixedQty fixes:

Not sure if anyone else is working in there. Give me the OK before I go and run 
the CGI reinstall script on it.

redmine issue

Yes, it can be. Did you check changes in M. 1.4.2.0 base template files?

Definitely our theme.
- Reinstalled /mageimage/ and got the correct checkout table on a M native 
product
- Installed the extension and checkout table got messed up
- disabled zptheme from system settings and refreshed the same page (checkout), 
table gets corrected
