feat: Sparse streams #41

BREAKING CHANGES:

Any ranges passed during construction of a BlockMap will be coerced to BlockMap.Ranges
Streams now emit objects of the shape { buffer, position } rather than Buffers with a monkey-patched .position property

TODO:

[x] Add doc block description for ReadRange
[x] Make ReadStream emit { buffer, position }
[x] Update README
