Reg\ Reg Operation - User enters whether Date of First Shipment has occurred

Description:

Post-MVP Task: add a new "yes/no" field to the operation information page

Context:

ED requested that whether Date of First Shipment has occurred be added to the registration fields (not immediately MVP, but at some point in the near future) on the Operation Information page. Reason: all regulated operations (not just new entrants) must provide whether they had a date of first shipment yet or not, so the BA knows whether they are eligible to receive a BORO ID. Generally, the answer will be "yes". If the answer is "no" it allows the BA to catch anybody who may be a new entrant but didn't select new entrant as their registration purpose.

Acceptance Criteria:

Given I am a regulated operation, new entrant, or opt-in, When I am on my operation information page on Register an Operation form Then I have a toggle that asks "Has this Operation had it's Date of First Shipment?"

(For context, no additional fields appears if the answer is yes or no)
