Define the Service specific elements of Permitify and define how to generate those elements

We'd like to create a tool (such as yeoman ) that will enable generation of a new Permitify-enabled service. Review Permitify and determine the steps for generating a new Permitify Service and the elements that one would manually tune for the specifics of the service. For example - defining the:

• Proof Requests for verification
• the credentials for issuance
• the data entry form that simulates the application
• the services.yml file for TOB integration
• the icon for the service
• perhaps a theme for the data entry form business

Document the list and perhaps some ideas for tools we could use for the data entry of each.

@ianco - I think you are mostly done this in the template documentation? Or do we still need to add this to do the documentation?
