Add ability to download/upload ccode tables as csv files in GUI

in config > User code types I have added a row count and upload/download 
buttons for each code list. New template for uploading.

Removed "plugin to read" text from UploadFileForm; avoid confusion since csv 
files are not plugins. Each option has a different upload page that indicates 
the type of file anyway.
