Document the process of discussing the ADRs proposal issues #50

The adr-process page says that an issue with a summary of what the ADR will be should be created to discuss and decide if the ADR is really desired or not.

There is however no info about what should happen next: should we ping someone? Should we send an email somewhere to kickstart the process?

Once the ADR/AP/PADR has been created, it's also not clear what the process is for reviewing/getting approval.
