Remove dead link to UserVoice from README
Move release notes from README to CHANGELOG
Added link from README to CHANGELOG
Add Dates to version headers in CHANGELOG (from commit history) for milestone releases
Add Index (table of contents) to CHANGELOG

@johnmcbride It seems maybe this repo is effectively dead. Should I just close this PR and delete my fork?
