Hide unused fields in Create New Lifeline #132

In the "Create a Lifeline" form hide the Source, Rate, and Citation Link fields as they are not currently used.

Would you prefer for me to comment this out? Or remove it completely from the code base? Personally, I'd advocate for the latter and if we need it again in the future we can retrieve from git history, but completely up to you.
