This got done iteratively. I started with the largest level, making sure that it was beatable, and then shrunk the player sprite down to the second largest size, and added gaps and platforms until it was beatable, and so on...
Probably the most significant element of this is the fish-tank, for which I had to add a swimming mechanic :) woop woop!
Design the rest of the game's levels based on the one created in #5
