as a user, I need to be able to add a new demographic record to the EMR #18

Demographics are needed to bill. To confirm to the Patient resource items listed in the OMD spec sheet, attached. EMR_Core_Data_Set_StandardCDS-S-_Data_Dictionary.xlsx

Screen should be inspired by the open source oscar-code based EMR, but modernized and adapted - keeping terminology matching would be ideal.

Initial setup to just contain first name and last name, in an area that can be used to add the other items.
