Allow token and url to be set via plugin options

Changes craftGqlUrl and craftGqlToken to be optionally set through plugin config options. Falls back to env variables.

closes #13

I was not able to compile the ts file without it exploding into a mess of changed lines. 😬

I instead opted to limit changes to keep things concise. Please let me know if I can help further.
